You understand and consent to receiving healthcare services via telehealth, which may include video, phone, or electronic communication.

Medical outcomes cannot be guaranteed. Treatment recommendations are based on the information provided during your visit.

You agree to provide accurate, complete, and truthful medical information to ensure safe and appropriate care.

Prescription Policy:
Prescriptions are provided at the provider’s discretion and in accordance with applicable laws and clinical judgment.

Brown NP in Family Health PLLC is not liable for any outcomes resulting from incomplete, inaccurate, or withheld information provided by the patient.
